Appointment Information
JPS Center for Cancer Care appointments are made by referral only. Below are the steps for the initial appointment at our center.
- A physician will refer you to JPS Center for Cancer Care.
- The New Patient Referral Coordinator (NPRC) will contact you to ask you preliminary questions about your diagnosis, insurance information, and answer and non-medical queries.
- You will need to provide the NPRC with the following information, which can be obtained or requested from the referring physician.
- Medical records containing:
- A diagnosis
- Current treatment status
- Copies of all x-rays and other films
- Copies of lab and pathology results
- Any additional material necessary to fully understand the case
- Full contact information on the referring physician and/or practice
- Full contact information on the patient
- Once the NPRC receives the above information, she will contact you to schedule your initial appointment.
Once your first appointment is scheduled, you will need to remember to bring the following items:
- Your current medications (bring the actual medication, NOT a list)
- Photo identification
- Member card for either your health insurance or Medicaid
If you do not have third party coverage listed above, you may need the assistance of our eligibility team. Please be able to provide:
- Social Security card
- Proof of residence
- Proof of household income
