Speak to an Enrollment and Eligibility Case Specialist

After your application is filled out and you have provided all of the supporting documents, an enrollment and eligibility specialist will contact you once an eligibility determination has been made or if additional information is required. He or she will ensure that JPS has all of the necessary information to process your membership.

If you submitted your application online or via postal mail or fax, please allow 10 days after receipt of all required documents to process your application. To check the status of your application, contact us at (817) 702-1001.

You may also print the application from the web site and call the JPS Enrollment & Eligibility Center at (817) 702-1001 to schedule an in-person financial screening appointment. Be sure to bring your completed application and all supporting documents to your scheduled appointment.

You are not a member of the assistance program until your application has been approved by a JPS enrollment and eligibility specialist. Your enrollment and eligibility specialist will help you through the application process and make sure you are enrolled in a timely manner.
